# Custom Incident Forms

You now have the option to create Custom Incident Forms.

What does “__Pre Release__” mean? 

The feature is released and available with improvements and more features under development.

A new Menu option has been created for Custom Incident Forms.

This will let you:

* Create and Publish Incident Templates
* Create your own questions using multiple question formats
* Easy input for Workers & Participants names / Incident Category and Severity
* Create Mandatory Fields
* View and Manage Active / Resolved Incidents
* Search, filter and Export for targeted reporting

## Permissions and Access

Admins can view and edit all Custom Incident Forms.

Coordinator’s can view and edit Incidents they created and for any Participant or Worker they are Assigned to.

If an incident doesn’t have a Participant or Worker attached, the Coordinator will have Read Only Access.


## Creating a new Custom Incident Form

This will display the existing template Name, Description, Created by, Status of Published or Not Published and Actions.

Select Create New Template

Add a Template Name and Description and Create Template

Select the Template and Add New Questions

You have multiple Question formats with different inputs available.

For each Question, add a Question Title / Question Description and choose if mandatory with a Response Required of Yes or No.

Now select the Response Type. Here are your options in List View:

You can only prompt for a Single response from:

Text Input / Incident Category / Incident Severity / Media - File Attachment / Signature Entry / Date / Time / Date & Time/ True or False / Number / Duration Picker


The other Response Types will allow you to prompt for Single or Multiple responses.


If you select Customisable Options, for Single or Multiple, you can type the available responses in the Selectable Options field and ‘+’ to keep adding more.

Once you have created all of your required responses, at the top of the page, press Publish. 

This will appear as available for input in the web portal and the worker app;

You can unpublish to and continue editing;

However once an input has been received, the form is no longer available for editing.

## Submit, View and Manage Incidents


From the Menu, select Custom Incident Forms and then Submitted Incidents.

1. To Submit a new Incident, select the + New Incident button

Select your form and Create Incident. Add in the different inputs depending on your selected form.

2. To View your incidents, use the basic filters. Choose a Template, add a date range and search by key phrase;

Once you have your relevant records, you can edit them 

There are 3 status’s of submitted incidents:  Received / In Progress / Resolved

You can edit each record as needed and enter any follow up notes or change the status.

3. To Export your Custom Incidents, press Export. Select your required template and Export Incidents.

   Each Custom Incident is exported separately as it will have different columns depending on it’s setup.

Your Export is now available and will appear similar to this:

## Worker App functionality

Your Workers can access the Custom Incident forms via the Incidents section on their device.

Then choose between the Original Incident Forms of the new Custom Incident Forms. Some user debrief may be required here.

Select the relevant Custom Form;

And input all of the requested fields, and Submit.

\*It will not let you submit the form unless all required fields are completed.

This will still send a notification to your nominated contact(s) based on the Worker Notifications.



## Notifications

CareMaster helps workers to be notified when incidents are submitted, when they change to in progress and when they are resolved. This is required for NDIS compliance reasons. This will apply only to custom incident forms (not legacy incident forms).


**Go to Settings > Worker Notifications**

When Custom Incident Forms is enabled for a provider, there are 6 templates configurable in the Worker Notification Setting. These can be sent via Email or SMS as per your default Preferences

**Custom Incident Received** - will send a confirmation email or SMS to the person who submitted a new custom incident

**Customer Incident in Progress** - will send a confirmation email or SMS to the person who submitted a new custom incident when the status of the incident changes to “In Progress”

**Customer Incident Resolved** - will send a confirmation email or SMS to the person who submitted a new custom incident when the status of the incident changes to “Resolved”

There are Notification Templates where you can personalise the message or add / remove variable fields.

On the Worker Notifications page scroll down till you fine the Custom Incident Notifications.

Free type to update the message.
